The historic centre radiates from Piazza del Duomo, one of Europe's largest cathedral squares and Milan's defining urban space. Within 15 minutes' walk: La Scala opera house (1778), the Galleria Vittorio Emanuele II (1877, Europe's oldest shopping arcade), the Quadrilatero della Moda (Via Montenapoleone, Via della Spiga — the most concentrated luxury retail district in the world), and the Pinacoteca Ambrosiana (Leonardo's Codex Atlanticus and Raphael's cartoon). The neighbourhood contains almost everything a first-time visitor to Milan needs to see, at the cost of being surrounded by other visitors doing the same thing.

- ↑The Duomo (construction 1386–1965, 135 spires, 3,400 statues) with rooftop terrace access: the view from the rooftop at sunset over a sea of Gothic pinnacles with the Alps visible to the north is the single most photographed view in northern Italy. Elevator access costs €13; stairs cost €9.
- ↑Galleria Vittorio Emanuele II — the 19th-century iron-and-glass arcade connecting Piazza del Duomo to Piazza della Scala — is both a working shopping mall (Prada, Louis Vuitton, Versace at ground level) and a masterpiece of structural engineering. The mosaic floor including the famous spinning-on-the-bull ritual is free to walk through at any hour.
- ↑La Scala opera house (booking via teatroallascala.org, tickets from €25 upper gallery to €200+ for prime positions): the world's most prestigious opera venue with a season running October–July. The La Scala Museum (entry €12) is accessible when the stage is in rehearsal.

- ↓The Duomo area has the highest tourist density in Lombardy: street vendor pressure, pickpocket risk in Piazza del Duomo, and queue management for the cathedral itself (90-minute waits in peak season without advance booking) make the immediate square best navigated early morning or after 19:00
- ↓Hotel prices in Centro Storico are the highest in Milan: comparable quality at 20–30% less within 5 minutes by Metro in Porta Nuova or Brera. The area around Piazza del Duomo has very few local restaurants — most dining options near the cathedral cater to tourists at tourist prices.
